The Transition Specialist works with young women in the Residential and Independence Place programs to assist them in developing independent living skills or transitioning to a different environment. Individual case management is provided including assistance with job search, apartment hunting, using public transportation and becoming familiar with community resources. The Transition Coordinator will take the time to assess strengths and address weakness’ overcoming barriers and working towards a successful transition to self-sufficiency.

As part of the transitioning process, life skills classes are available to any clients engaged in Residential or Community programs through LFC. Classes are taught by a multi-disciplinary team and utilize the knowledge and strengths of a diverse team of treatment providers. Life Skill classes incorporate gender and age specific approaches to life skill acquisition as well how parenting affects all aspects of life.

Classes offered can include:

  • College and Career exploration
  • Goal Development and planning
  • Time Management
  • Organization
  • Health education and management
  • Communication and Advocacy Skills
  • Relationships
  • Job readiness
  • Living on your own
  • Community Resources
  • Kitchen Basics

Women in the Residential program also develop life skills through practical applications such as: daily cleaning, preparing meals, laundry, participation on a residential council, managing a weekly allowance, time management, formal education attendance, community service, job search and acquisition. Ultimately leading to maintaining employment and financial planning/money management. Residential Counselors assist in skill development in these areas and residents have access to 24-hour assistance, support, education, and guidance through residential staff. The Transition Coordinator works closely with the on-site Reach Up workers to ensure compliance with Reach Up requirements while also engaging in necessary individualized treatment services.

Women living at Independence Place also work with the Transition Coordinator and IP staff to develop and strengthen necessary skills for successful independent living. The Transition Coordinator recommends eligible women from the Residential programs that are prepared to move on to Independence Place and can help other eligible women in the community with the referral and intake process. Tenants are assessed when they move in to better guide the programming and/or services they may utilize while living at IP. Tenants are also guided by the Family Development Plan they create with their Reach Up worker and can utilize the support of the IP staff and Transition Coordinator to comply with their Reach Up requirements.

Replication Project

Women who are currently enrolled in any of Lund’s programs have the ability to participate in the Replication Project. This project recognizes women who show motivation, responsibility, and are in need of specific job training/job experience. LFC provides an “apprenticeship” position to already existing positions at LFC allowing women to engage in on-the-job training. Replication participants receive a small stipend and on occasion, also receive larger financial incentives for completing work goals and receiving favorable evaluations. Participants can utilize on site support and services if experiencing problems at home or on the job that could interfere with their continued employment. These services assist women in learning important skills to maintain employment. Participants are also encouraged to attend weekly life skill classes that specifically address job readiness and career planning as a part of their experience in the Replication Project. Upon successful completion, participants have a strong employee recommendation as well as job experience to put into a resume.
