Transitional Services

The Transition Specialist works with young women in the Residential and Independence Place programs to assist them in developing independent living skills or transitioning to a different environment. Individual case management is provided including assistance with job search, apartment hunting, using public transportation and becoming familiar with community resources. The Transition Coordinator will take the time to assess strengths and address weakness’ overcoming barriers and working towards a successful transition to self-sufficiency.

As part of the transitioning process, life skills classes are available to any clients engaged in Residential or Community programs through LFC. Classes are taught by a multi-disciplinary team and utilize the knowledge and strengths of a diverse team of treatment providers. Life Skill classes incorporate gender and age specific approaches to life skill acquisition as well how parenting affects all aspects of life.

Classes offered can include:

Women in the residential program also develop life skills through practical applications such as: daily cleaning, preparing meals, laundry, participation on a residential council, managing a weekly allowance, time management, formal education attendance, community service, job search and acquisition. Ultimately leading to maintaining employment and financial planning/money management.

Residential Counselors assist in skill development in these areas and residents have access to 24-hour assistance, support, education, and guidance through residential staff.

The Transition Coordinator works with women receiving transition case management, Reach Up Services and women in the Replication Program, a job skills training program all of which are offered at Lund Family Center.


Reach Up

The Reach Up program is a welfare to work program which serves parenting women from our residential program and the community at large. There is no age limit on women who receive services from the residential programming at Lund; parents who are living in the community and receive Reach Up services must be under the age of 25. All minor parents in Chittenden County who are eligible for Reach Up, receive their Reach Up case management at Lund Family Center. For more on the Reach Up program, please click here.


Replication - A Job Skills Training Program

The Replication Project started several years ago at Lund as an on-site job training program for Lund Family Center parents. Participants work with Lund staff in a variety of different job sites to learn job skills specific to their field of interest. For more on the Replication project, please click here.
